Under the Influence book review The Sober Curator

“Under the Influence: A Guide to the Myths and Realities of Alcoholism” by James R. Milam and Katherine Ketcham is a seminal work that delves into the complexities of alcoholism, offering a comprehensive analysis that dispels common myths and provides valuable insights for both addicts and their families. It’s been the core “textbook” of sorts for the Lakeside Recovery Treatment Center. As a person in long-term recovery myself, I have read this several times and given copies out to friends and doctors who want to understand more about the disease of alcoholism. (Sober Curator Pro Tip: Be sure to order the latest version for the most current statistics since this book has been around for decades.) 

Let me explain how this book is laid out:  

Overview and Structure 

The book is divided into three main parts: 

  1. Understanding Alcoholism 
  2. The Alcoholic Family 
  3. Recovery and Treatment 

So, each part is meticulously crafted to address different aspects of alcoholism, providing readers with a holistic understanding of the disease and its impact. 

Part 1: Understanding Alcoholism 

Chapters 1-4: The Nature of Alcoholism 

Milam begins by defining alcoholism as a chronic, progressive, and potentially fatal disease. He emphasizes that alcoholism is not a result of moral weakness or a lack of willpower but a biochemical disease that affects the brain’s functioning. This section is always a little over my head each time I re-read it, but you’ll love it if you’re a details person. Under the Influence does a great job of highlighting the bottom line, which is this: it’s crucial for addicts and alcoholics to understand that their condition is a medical issue, not a personal failure. 

Key Insights: 

  • Alcoholism involves a biochemical imbalance in the brain, which leads to a compulsive need for alcohol. 
  • The disease follows a predictable progression, which can be halted with proper treatment. 
  • Understanding the medical nature of alcoholism can help reduce the stigma and shame associated with it. 

Chapters 5-8: Myths and Realities 

This section debunks several myths about alcoholism. For instance, the authors clarify that alcoholism can affect anyone, regardless of social status or background. They also explain that moderate drinking is not necessarily safe for everyone, as some individuals are more genetically predisposed to alcoholism. 

Key Insights: 

• Alcoholism is not limited to a specific demographic; it can affect anyone. 

• Genetic factors play a significant role in the development of alcoholism. 

• Early intervention and education are crucial in preventing the progression of the disease. 

Part 2: The Alcoholic Family 

Chapters 9-12: Dynamics of the Alcoholic Family 

Milam and Ketcham explore how alcoholism affects not only the individual but also their family. They describe the roles family members often assume in an attempt to cope with the alcoholic’s behavior, such as the enabler, the hero, the scapegoat, and the lost child. Understanding these dynamics is essential for families to break the cycle of codependency and seek appropriate help. 

Key Insights: 
  • Family members often adopt specific roles that enable the alcoholic’s behavior. 
  • Codependency is a common issue in families affected by alcoholism. 
  • Family therapy and support groups are critical for healing and recovery. 

Chapters 13-16: Breaking the Cycle 

So, this section provides strategies for family members to break free from their enabling roles and encourage the alcoholic to seek treatment. The authors stress the importance of setting boundaries, seeking professional help, and joining support groups like Al-Anon. 

Key Insights: 
  • Setting firm boundaries is crucial for the family’s well-being. 
  • Professional counseling can help families navigate the complexities of addiction. 
  • Support groups offer a safe space for sharing experiences and gaining support.

Part 3: Recovery and Treatment 

Chapters 17-20: Treatment Options 

Milam and Ketcham outline various treatment options available for alcoholics, including detoxification, inpatient rehabilitation, outpatient programs, and support groups like Alcoholics Anonymous (AA). They highlight the importance of a comprehensive treatment plan that addresses both the physical and psychological aspects of addiction. They have multiple facilities around the Seattle, WA area. (Sidebar: Yours truly took an outpatient tour there for a few months, and I read this book cover to cover several times, clinging to the comforting mantra that my alcoholism was not my fault.)  

Key Insights: 
  • A combination of medical treatment and counseling is most effective for recovery. 
  • Support groups like AA provide a strong network for maintaining sobriety. 
  • Relapse is a common part of the recovery process and should be addressed with compassion and support. 

Chapters 21-24: Long-term Recovery 

The final chapters focus on the long-term aspects of recovery, emphasizing the need for ongoing support and lifestyle changes. The authors discuss the importance of building a sober support network, finding healthy coping mechanisms, and continuing therapy as needed. These are the basics that are more than basics… more like essentials. 

Key Insights: 
  • Long-term recovery requires ongoing effort and support. 
  • Building a solid support network is crucial for maintaining sobriety. 
  • Healthy lifestyle changes and coping strategies are essential for preventing relapse. 
Importance for Addicts and Alcoholics 

The reason I have given copies of this book out so many times is chiefly because it explains the disease of addiction far better than I ever could. I believe “Under the Influence” is pretty much a vital resource for addicts and alcoholics for the following reasons:  

  1. Medical Understanding: It provides a scientific explanation of alcoholism, helping individuals understand that their condition is a medical disease rather than a personal failure. 
  2. Breaking Stigma: By addressing the myths and realities of alcoholism, the book helps reduce the stigma associated with addiction. 
  3. Family Dynamics: It offers valuable insights into how alcoholism affects family members and provides strategies for breaking the cycle of codependency. 
  4. Comprehensive Treatment: The book outlines various treatment options and emphasizes the importance of a holistic approach to recovery. 
  5. Long-term Support: It highlights the need for ongoing support and lifestyle changes to maintain sobriety. 
Conclusion 

“Under the Influence” by James R. Milam and Katherine Ketcham isn’t the page-turner of addiction books, and make no mistake, it’s not quite lit, and you’ll be disappointed if you assume it is. However, it IS, I think, an essential read for anyone affected by alcoholism. Its thorough examination of the disease, combined with practical advice for recovery, makes it a powerful tool for addicts, their families, and professionals in the field of addiction treatment. By demystifying alcoholism and offering clear, actionable steps for recovery, this book provides hope and guidance for those struggling with this challenging disease. And if you’re one of us, it makes you feel like you’re not alone, and it’s not your fault. Personally, I think it’s my responsibility to treat my disease, and reading this book was one of the crucial first steps.
